5th Grade Vaccine Clinic on 04/26 at JES! Forms must be returned by 04/19 if you wish for your student to receive the vaccine. The Tdap is a MUST for all students entering the 6th grade next year. Your student must be 11 by the date of the clinic which is 4/26/04 to participate. This is a GREAT opportunity!
